Registered Nurse (RN) Case Manager

The RN Case Manager plans, organizes and directs home care services, building from community resources to meet the needs of individuals and families within their homes and communities. Our team strives to provide quality, safe and reliable care to community-based patients in their own homes to provide a holistic one-on-one approach between staff and patient.

Qualifications:

• Graduate of an accredited school of nursing
• Bachelor’s degree, with one (1) year of home health care experience preferred.
• Active RN or LPN license in Iowa
• AHA BLS/CPR certification
• Driver’s license and an insured, working automobile
